MPlayer, a couple of Perl scripts, an imageviewer and a bit of time. I look through the movies for interesting sections, run those sections with MPlayer's dump-to-disk video output mode and finally look through all the frames for the ones I like. It's more work than just viewing and hitting a "Capture" button on the fly, but it allows me to get those things that are only visible for two or three frames (like this picture of Gabrielle (http://www2.cyberpomo.com/screencaps/XenaS3/s3e17_c2_00000071.jpg), which I still think is actually Renee, since her usual double is much less muscular).
I don't know if MPlayer runs on Windows at all (I use FreeBSD) and the Perl scripts aren't really usable by anyone but me, so I doubt anyone else could use my exact setup.
