Where do you usually think up your ideas?
Anywhere, any time. They just come. In droves. I get far more ideas than I'll ever have the time (or energy) to write.
Are drugs and/or alcohol part of your 'creative process'?
Only in that alcohol utterly kills my creativity. Can't do anything that requires making stuff up while even very slightly drunk. I've tried writing while drunk, and the result was dismal. I've tried gamemastering roleplaying games while drunk, and the only reason it wasn't a total catastrophe was that the players were even more under the influence. I've tried doing improv radio after having had a cup or two of mulled wine, and it was scary how much harder than usual it was.
What do your fics start out as?
Most often, an entire scene from somewhere in the middle of the story flashes into my mind in its entirety, complete with visuals, dialogue and special effects. This scene will almost never be in the finished story.

It's so much less work when you already have a background and a set of characters.
